Advance Care Planning is making decisions about the type of care you would like if you are ever unable to speak for yourself. Collect important information and answers to questions while making a deliberate plan. Complete your advance care planning decisions and share those decisions with people in the position of making determinations for you. Learn how and why you should explain what’s important to you. Initiate decision-making conversations with your loved ones where you may be the one in charge. Includes an overview of the Frederick Memorial Hospital (FMH) Advance Care Planning Initiative. It is never to early to plan. Do it for yourself and your loved ones.
